Inappropriate use of perioperative antibiotics is a major driver of antimicrobial resistance (AMR), emphasizing the need for effective antimicrobial stewardship strategies in surgical settings.

Objective: This review aims to synthesize current evidence on perioperative antimicrobial stewardship interventions designed to optimize surgical antibiotic prophylaxis and mitigate antimicrobial resistance.

Conclusion: Perioperative antimicrobial stewardship represents a critical strategy for optimizing antibiotic use in surgical care and addressing the global AMR crisis. Future research should focus on integrating digital health technologies, improving stewardship implementation in resource-limited settings, and evaluating long-term impacts on antimicrobial resistance and surgical outcomes.

Keywords: Surgical antibiotic prophylaxis, antimicrobial stewardship, surgical site infection, antimicrobial resistance, perioperative care.
