- <div> <div><strong>Wrong is the application of any operation before its prerequisite has produced its output</strong>. An operation that occurs without its prerequisite's output receives no valid input, and the failure to close is the structural signature of error. The four operations correspond to the four foundations of mathematics, logic for distinction (*On Occurrence*, Stewart, 2026g), set theory for placement (Stewart, 2026g), type theory for identification (Stewart, 2026g), and category theory for composition (Stewart, 2026g), with the directed-graph structure of the ordering derived in *The Endomorphic Collapse: The Path* (Stewart, 2026e). The present paper assumes only the operational dependency and develops its consequences for error.</div> <br> <div><strong>Wrong, as experienced, is expectancy violation, the divergence between a projected expectation and the actual determination.</strong> Expectancy violation follows from the ordering. <strong>Expectation requires ordering, so a system without one has nothing to project forward to and cannot expect anything.</strong> Several phenomena that resemble structural error are not. Incompleteness (an ordering that stalls without attempting a later operation) is the clearest example. It is not a structural failure mode. It appears as wrong only in the observer's expectancy violation. The structural classification covers all and only the misorderings.</div> <br> <div><strong>Structural errors classify into five non-empty classes plus one structurally empty cell</strong>, with the count forced by Q's edge set (Stewart, 2026g) and the three closed paths from L back to L derived at the path-algebra scale (Stewart, 2026e; *The Path Algebra of the Endomorphism*, Stewart, 2026f). The three closed paths are named γ₁ γ₂ γ₃ at the substrate scale. At the error-scale paper's own scale, γ₁ is the full four-operation ordering, the L → S → T → C → L path that runs every position in sequence. γ₂ is the direct-admission path, the L → T → C → L path that delivers distinction's output to identification through the direct edge a₅ without passing through placement. γ₃ is the apex-and-commitment-return path, the T → C → T path between identification (the apex's operation, producing the determinate identity) and composition (the commitment that carries forward). The five non-empty violation classes correspond to specific edge-failures within these three paths. Class 2-1 is the a₁ edge-failure within γ₁. Class 3-2 is the a₂ edge-failure within γ₁. Class 4-3 is the a₃ edge-failure, which sits in all three paths. Class 3-1 is the a₅ edge-failure within γ₂. Class 4-1 is γ₁'s catastrophic failure mode where the full four-operation ordering does not complete. The sixth potential class (Class 4-2, the S-C reversal) is structurally empty because Q has no S-C edge in either direction and the endomorphism's closure forbids the operation it would require. Each class is named by the pair (L, E), where L is the later operation that occurred without its prerequisite having delivered and E is the earlier prerequisite, with Q locating the failure at a specific edge within a specific closed path rather than as a combinatorial pair-reversal. The definition operates uniformly wherever the four-operation ordering runs. A conclusion reached without premises established is composition without distinction, the same misordering as an effect without its cause in physics, a null reference in computation, an action without its perceptual determination in cognition, and an injustice that operates on a subject whose recognition was never established in ethics. The structural form is the same. The instances differ in what the operations are running on. Clinical conditions (impulsivity, depression, panic disorder, hallucination, PTSD, generalized anxiety) instantiate specific violation classes with predicted neural timing signatures. The causal structure of spacetime is the operational ordering applied to events. The historical development of programming languages is the progressive enforcement of the ordering from runtime to compile time. In ethics, injustice is composition before the recognition of a subject's existence, with two sides to the misordering. The agent's ordering continues forward through the misordering. The subject is operated on without the prerequisite (their recognition as a subject) having been established, so the subject's ordering is broken from outside. Both sides inhabit the same malformed ordering. The form of the failure is symmetric, the consequence is not.</div> <br> <div><strong>Error requires at least two operations and the possibility of misordering them. Where only one operation remains operative, no ordering remains to violate.</strong> The first operation is the fixed point of the error structure. It cannot be displaced by any misordering, because every misordering displaces a later operation relative to an earlier one. The first operation can never be the late one in such a misordering, because no operation precedes it.
