- Purpose: Accessing gender-affirming surgical care remains fragmented and challenging for transgender and gender-diverse individuals. Barriers include limited provider directories, inconsistent insurance coverage navigation, misinformation, and difficulty coordinating perioperative expectations across multiple specialties. These challenges contribute to delayed care, patient dissatisfaction, and disparities in outcomes. Digital solutions tailored to the transgender community are lacking within the field of plastic and reconstructive surgery. We present Transcend , a novel digital platform designed to streamline the patient journey in gender-affirming care. The platform aims to centralize provider information, improve health literacy, and empower patients with personalized resources and tools to enhance surgical preparedness and postoperative recovery. Methods: The app was developed using an iterative design process incorporating patient interviews, stakeholder feedback, and consultation with gender-affirming surgeons at UCLA and Northwell Health. Core features include: (1) a color-coded calendar for managing repeat appointments, multiple specialists, and recovery milestones; (2) a gender affirming provider directory with reviews to ensure safety and trust; (3) educational modules detailing pre- and postoperative expectations for facial, chest, and genital gender affirming surgeries; and (4) an AI-powered assistant to address common patient questions and direct users to appropriate resources. Results: Prototype testing with pilot users within UCLA Health demonstrated improved confidence in navigating surgical planning, reduced anxiety regarding perioperative steps, and strong interest in the provider network for referrals. Feedback emphasized the value of centralized, trustworthy resources and the ability to individualize surgical preparation. Conclusions: Transcend represents a first-of-its-kind digital platform to address barriers in gender-affirming surgical care. By combining provider verification, patient education, organizational tools, and AI support, the platform offers a scalable and patient-centered model.
