This working paper furnishes an explanation of: (1) the background and preparation of hospital library survey forms used by the Ohio Valley Regional Medical Program, University of Louisville division, and (2) the procedure used by the University of Louisville for surveying hospital libraries within the Ohio Valley Regional Medical Program area.
