Appendices include: (1) New York Library Association forms, including a self-censorship quiz, membership applications, an incident report form, and a formal request for assistance; (2) three sample policies, one each from an academic, public, and school library in New York State; and (3) American Library Association policy documents, including, among other things, the Freedom to Read Statement and the Library Bill of Rights.
