Over the past few years, therehave been advances in technology, such as the developmentof nucleic acid amplification assays, which have improvedour ability to diagnose infections caused by Chlamydia trachomatis.The finding that nucleic acid amplification tests candetect more infected individuals and are useful in screeninglow prevalence populations, has led to the development ofstrategies designed to reduce the cost of these assays withoutsignificantly impacting their sensitivity. The developmentof new tests for the diagnosis of syphilis has gained momentumfrom the report of a synthetic VDRL antigen, whichwill result in better nontreponemal antibody tests for syphilis.In spite of the completion of the genome sequence ofTreponema pallidum and its annotation, we are still unableto cultivate this microorganism in vitro. However, the molecularrevolution has resulted in the development of PCRassays for detecting Treponema pallidum in various types ofclinical specimens, and to the production of recombinantantigens for use in tests that detect treponemal-specificantibodies. Further research will improve the availability oflow cost, sensitive tests for the diagnosis of sexually transmittedinfections.
