The vast majority are secondary to trauma or surgery in the temporal region and 95% of cases progress to pseudoaneurysms. However, true or spontaneous aneurysms are extremely rare, accounting for 8% of cases of superficial temporal artery aneurysms, and are usually caused by atherosclerosis. Spontaneous temporal aneurysms can coexist with other vascular lesions, including intracranial aneurysms.
