The compromise of the bile duct usually occurs in portal obstruction due to the cavernous transformation of the portal vein (CTPV).We present a case of a young patient with a recent history of portal hypertension and CTPV who presented with an episode of cholestatic hepatitis. Studies documented an image of nodular appearance with extrinsic compression of the distal bile duct compatible with a tumor-like cavernoma.
