| contents | Edema cerebral: aportaciones de la investigación básica al manejo clínico Karina Tuz Herminia Pasantes Morales Biología hyponatremia cerebral edema cytotoxic edema cell volume regulation Hyponatremia correction Brain edema is a severe clinical complication in numerous pathologies and may result in neuronal damage and death. Brainedema may be of cellular or vasogenic type, although in most pathologies both types coexist. Cellular edema predominantlyoccurs in astrocytes. It develops in anisosmotic or in isosmotic conditions. The anisosmotic (hyposmotic) edema associateswith hyponatremia and the isosmotic edema is present in epilepsies, head trauma, ischemia and hepatic encephalopathy.Hyposmotic cellular edema can be actively reversed by mechanisms of cell volume adjustment, accomplished by the extrusionof intracellular osmolytes, namely K+, Cl- and organic molecules, predominantly amino acids, polyalcohols and amines.Osmolyte efflux in hyponatremia decreases the intracellular concentration of solutes and leads to a cell/plasma osmoticequilibrium. These adaptive mechanisms must be considered during clinical correction of hyponatremia, whose pace shouldbe in accordance to the cell adaptation again to a normal isosmotic environment. The mechanisms of cell volume adaptationdo not efficiently operate in isosmotic edema, most likely due to the fact that ionic distribution is responsible for the cell swellingand consequently, ions cannot participate as corrective elements. Therefore, a prevention strategy seems to be more adequateto counteract the edema associated damage 2004 artículo científico 1405-888X https://www.redalyc.org/articulo.oa?id=43270205 en http://www.redalyc.org/revista.oa?id=432 Tip Revista Especializada en Ciencias Químico-Biológicas application/pdf Universidad Nacional Autónoma de México Tip Revista Especializada en Ciencias Químico-Biológicas (México) Num.2 Vol.7 |
