In addition tobehavioral modifications, first-line treatment optionsinclude nicotine replacement therapies andbupropion SR. Varenicline, which was recentlyapproved by the Food and Drug Administration(FDA), offers a novel mechanism of action forsmoking cessation. This article reviews current firstlinesmoking cessation aids and evaluates theclinical trials pertaining to the efficacy and safety ofvarenicline. Additionally, the authors attempt toestablish the role of varenicline in smokingcessation therapy and determine whethervarenicline should be used prior to other first-linesmoking cessation aids, particularly considering thelower costs of generic alternatives. At present,clinical studies have not established the efficacy ofvarenicline after repeated courses, followingbupropion failures, or in various unstudiedpopulations.
