Determining the barriers to theimplementation of these practice guidelines mayhelp improve patient care.Objective: To determine whether barriers to the useof pharmacotherapy according to practiceguidelines are related primarily to patient orprescriber factors.Methods: Retrospective cohort study. Members of ahealth maintenance organization identified ashaving spirometry-defined COPD ranging fromstage II to IV. Electronic medical records werereviewed for documentation of the following: 1)patient affordability issues, 2) history of an adversedrug reaction, 3) history of inefficacy to therapy, and4) prescription history.Results: A total of 111 medical records werereviewed. There were 51% of patients who had notfilled medications that had been prescribed inaccordance with guidelines and 43% did not havethe guideline recommended medications prescribedin the previous year. Only 4% and 2% of patientshad documented inefficacy and affordability issues,respectively. There were no reported cases ofadverse drug reactions.Conclusions: This study provides insight to theacceptance of COPD treatment recommendationsby patients and providers.
