Despite being thepreferred choice for uncomplicated essentialhypertension, thiazide diuretics continue to beunderutilized.Methods: Uncomplicated essential hypertensionpatients taking a single antihypertensive medicationwere evaluated upon enrollment, diagnosis afterenrollment or initiation of therapy in treatment naïvepatients. Clinician prescribing habits weredetermined for both pre-existing and newlydiagnosed hypertensive patients. For the costsavings analysis, hydrochlorothiazide (HCTZ) 25mgdaily was selected as the preferred conversionmedication.Results: Four hundred seventy-eight patients wereincluded. ACE inhibitors were the most prescribedat 35.4% (n=169), followed by dihydropyridinecalcium channel blockers (DHP CCB) and thiazidediuretics, both at 20.3% (n=97). Only 12.9% (n=33)of patients with hypertension that were taking anantihypertensive medication upon enrollment wereeither continued or started on thiazide diuretictherapy. Newly diagnosed or treatment naïvepatients were prescribed a thiazide diuretic 28.8%(n=64) of the time. DHP CCB accounted for 58.8%of the total medication cost per month with thiazidediuretics responsible for 0.8% of the cost. If allpatients had been prescribed HCTZ 25mg daily,95.8% of the total medication cost per month couldhave been saved.Conclusions: Thiazide diuretics were underutilizedas preferred therapy in patients with pre-existing ornewly diagnosed uncomplicated essentialhypertension.
