  • Fetal Sylvian Fissure Maturation in Congenital Heart Disease and Its Relation to Expected Brain Arterial Oxygen Saturation: A Longitudinal Study Xiaowei Xiong Yuanjie Cui Chenxiao Hou Lina Yi Shangjin Zhong Na Zhang Qingqing Wu Prenatal Diagnosis ABSTRACT Objectives To compare fetal sylvian fissure (SF) maturation between normal controls and fetuses with congenital heart disease (CHD), and to investigate the relationship between SF development and expected cerebral arterial oxygen saturation (SaO 2 ) levels. Methods This was a retrospective longitudinal study in singleton pregnancies, including 442 cases with CHD and 446 normal fetuses. Fetuses with isolated CHD were further categorized into two groups according to the expected level of cerebral arterial SaO 2 : normal to mildly reduced and moderate to severely reduced. Fetal ultrasound images obtained after 19 weeks' gestational age (GA) were analyzed. Measurements included biparietal diameter (BPD), head circumference (HC), SF depth, and insula depth. Results Significant differences in SF depth/insula depth ratio were observed between isolated and different subtypes of nonisolated CHD ( P FDR < 0.01). The SF depth/insula depth ratio demonstrated a consistently lower trend in the isolated CHD group than in the control group, with a progressive reduction as cerebral arterial SaO 2 worsened in the CHD cohort ( P FDR < 0.001). Conclusions Delayed SF development in fetuses with CHD may be associated with the expected cerebral arterial SaO 2 reduction. Prenatal cortical folding can be reliably assessed by the SF depth/insula depth ratio or the SF depth/HC ratio. 10.1002/pd.70100 http://onlinelibrary.wiley.com/termsAndConditions#vor
