contents
Early detection of COVID-19-associated pulmonary aspergillosis (CAPA) in critically ill patients remains a significant challenge, with the lack of clear diagnostic criteria contributing to high mortality rates. This study evaluates the utility of tracheal aspirate for the early diagnosis of CAPA in intensive care unit (ICU) patients and introduces the concept of "CAPA in Progress" (CAPA-IP), based on the quantification of galactomannan (GM) levels. By establishing a novel diagnostic threshold for GM in tracheal aspirate, this approach provides a less invasive alternative to bronchoalveolar lavage (BAL), facilitating earlier antifungal intervention and reducing mortality. The methodological innovation of this study lies in the validation of this diagnostic threshold in a prospective cohort, demonstrating its efficacy in optimizing antifungal treatment in critically ill patients. Given the urgent need for improved secondary infection management in COVID-19 patients with acute respiratory distress syndrome (ARDS), this study addresses a critical gap in clinical practice and enhances diagnostic capabilities for opportunistic fungal infections in the ICU.
