contents <p>Prediabetes is a state of intermediate hyperglycaemia. It remains a high-risk state for developing diabetes mellitus which resembles the concept of <em>Prameha</em> in Ayurveda. Prediabetes may be correlated with <em>Prameha purvarupa, </em>and they arrive as an opportunity to act early. In Ayurveda, the goal of Prediabetic management is to normalise the blood sugar level by giving attention to three aspects - <em>Aushadha</em> (Medicine), <em>Ahara</em> (Diet) and <em>Vyayama</em> (Exercise). This study is to assess the effect of a Medicated <em>yavagu </em>and Lifestyle modification in Prediabetes. This Medicated <em>yavagu </em>is mentioned in the Kerala’s traditional therapeutic treatise Yogamrutam,<em> Prameha chikitsa.</em> It is prepared with <em>Raktasali</em> <em>(Oryza sativa)</em> and <em>Mudga</em> <em>(Vigna radiata)</em> in <em>kashaya</em> of 8 herbal ingredients-<em>Paaranthi</em> root <em>(Ixora coccinea)</em>, <em>Khadira sara</em> <em>(Acacia catechu)</em>, <em>Bhadra</em> root <em>(Aerva lanata)</em>, <em>Saalaparni</em> root <em>(Pseudarthria viscida),</em> <em>Dusparsa</em> root <em>(Tragia involucrata)</em>, <em>Vasukah</em> root <em>(Spermacoce hispida)</em>, <em>Daaruharidra </em>stem <em>(Coscinium fenestratum)</em> and <em>Ghotika</em> root <em>(Portulaca oleracea)</em>. A Pre-post Interventional study was done in 60 participants with 30 each randomly allocated to Intervention group (Group I) and Comparison group (Group II). In Group I, Medicated <em>yavagu</em> was administered along with Lifestyle modification which includes dietary restriction and exercise. In Group II, Lifestyle modification alone was advised. The study period was 30 days. FBS and PPBS levels were analyzed on 0<sup>th</sup>, 15<sup>th</sup> and 31<sup>st</sup> day. The study was analyzed statistically using paired t-test, student’s t-test and repeated measures ANOVA. After intervention, both Group I and II elicited statistically highly significant reduction in FBS and PPBS values (p<0.001). When comparing both groups, the effect was statistically highly significant (p<0.001) in reducing FBS and PPBS levels. The results indicates that Medicated <em>yavagu</em> along with Lifestyle modification is more effective than Lifestyle modification alone in Prediabetes.</p>
