  • <p>The establishment of <a href="https://ijetrm.com/issues/files/Jul-2024-05-1751716584-OCT202409.pdf" target="_blank" rel="noopener">medically integrated dispensaries (MIDs)</a> within clinical settings represents a transformative<br>model in U.S. outpatient care, enabling real-time access to medication, personalized pharmacist consultations, and<br>improved therapeutic adherence. This paper explores the operational, regulatory, and technological landscape of<br>clinic-owned MIDs, focusing on their implications for value-based care and patient-centric service delivery. While<br>these models are gaining traction in oncology, rheumatology, and chronic disease management clinics, their<br>adoption is influenced by complex state-level pharmacy regulations, reimbursement constraints, and EHR<br>interoperability challenges. The first section analyzes the regulatory frameworks that govern MIDs, including<br>state pharmacy board licensure requirements, Medicare Part D considerations, and compliance with Drug Supply<br>Chain Security Act (DSCSA) mandates. The paper then explores the role of digital integration, highlighting how<br>electronic prescribing, inventory management, and real-time benefit tools enable seamless clinical-pharmacy<br>communication and reduce medication errors. Emphasis is placed on the technical architecture required for EHR–<br>dispensary interoperability and data-driven reporting. Through a cost-benefit lens, the study presents evidence<br>from health systems and specialty clinics where MIDs have led to increased medication adherence, lower total<br>cost of care, and reduced patient abandonment rates. It also evaluates risks, including conflicts of interest and<br>potential overutilization. The analysis concludes with a framework for implementing MIDs in diverse care<br>settings, recommending cross-disciplinary collaboration, digital maturity alignment, and robust compliance<br>protocols. Ultimately, clinic-owned MIDs serve as a strategic enabler of patient-centered care, especially in<br>chronic and high-acuity therapeutic areas</p>
