  • <p><strong>ABSTRACT</strong></p> <p><strong>Background:</strong> Chronic obstructive pulmonary disease (COPD)is a progressive respiratory disorder characterized by airflow limitation and increased inflammatory response. Individuals with COPD experience early exertion, necessitating an assessment of physical activity levels (PAL)and their correlation with Rating of Perceived Exertion (RPE) and Peak Expiratory Flow Rate (PEFR).</p> <p><strong>Methods:</strong> A total of 92 individuals aged 40–80years with mild to moderate COPD were recruited based on inclusion criteria in this one-time observational study. Participants performed PEFR, the Short Physical Performance Battery (SPPB), and the 2-Minute Walk Test (2MWT). Borg's RPE scale was used to assess exertion levels posttests. Data was analyzed using R software (v4 1 2) and Microsoft Excel. A p< 0.05 was statistically significant.</p> <p><strong>Result:</strong> Data was analyzed using R software (v4 1 2) and Microsoft Excel. Normality was confirmed by the Shapiro-Wilk test, and parametric tests were applied accordingly. The chi-square test showed no significant association between age group and gender (p = 0.248). Regression analysis revealed that PEFR, 2-minute walk distance, and RPE were statistically significant predictors of SPPB scores across all age groups. PEFR consistently demonstrated the strongest predictive value, with the highest effect in the 70–80 years group (t= 8. 98, p = 0. 0001, R² = 0. 778). While the 2-minute walk distance was significant in all groups, its direction varied in younger cohorts. RPE was also statistically significant but contributed less to the explained variance, indicating limited clinical relevance.</p> <p><strong>Discussion:</strong> Age-wise regression analysis showed that PEFR was a consistent and strong predictor of SPPB scores across all age groups, with its highest predictive value in the 70–80-year group. The 2-minute walk distances showed consistent directionality in younger groups but emerged as a meaningful positive predictor in older adults. RPE was statistically significant across age groups but had limited clinical relevance, especially in younger participants. These results emphasize the importance of pulmonary function and endurance in evaluating age-related functional performance decline.</p> <p><strong>Conclusion:</strong> PEFR emerged as a strong predictor of physical performance across all age groups, with predictive strength increasing with age. The 2-minute walk distance was a significant predictor in older adults, while RPE, though statistically significant, had limited clinical relevance.
