•
Viral exanthems are common dermatological manifestations in pediatric patients, often posing diagnostic challenges due to overlapping clinical features. Accurate identification is crucial for timely management, prevention of complications, and public health interventions, particularly in India, where viral infections such as measles, rubella, chickenpox, and enteroviral infections remain prevalent. This article provides a practical guide for Indian clinicians, focusing on the differential diagnosis of viral exanthems in children. It outlines the epidemiology, pathogenesis, characteristic clinical presentations, and complications of major viral exanthems. Emphasis is placed on structured diagnostic approaches, including thorough history taking, detailed physical examination, and judicious use of laboratory investigations. Diagnostic algorithms, flowcharts, and illustrative clinical images are incorporated to facilitate rapid and accurate recognition. This guide aims to enhance clinical decision-making, reduce misdiagnosis, and improve patient outcomes in pediatric practice.
