- <p>Background<br>Pyrexia of unknown origin (PUO) continues to represent a complex diagnostic problem in adult medicine, particularly when initial empirical antimicrobial therapy fails to produce clinical improvement. Inflammatory endocrine conditions are often under-recognized contributors to prolonged febrile illness.</p> <p>Case Overview<br>This scholarly clinical analysis presents a middle-aged man with prolonged fever, tachycardia, unintentional weight loss, and anterior neck pain. A focused clinical evaluation, supported by targeted thyroid function testing, was consistent with subacute (De Quervain) thyroiditis. The clinical course highlights the diagnostic value of careful symptom correlation and selective investigation in patients presenting with PUO.</p> <p>Educational Value<br>The case emphasizes the importance of including inflammatory thyroid disorders in the differential diagnosis of PUO and demonstrates how timely recognition may prevent unnecessary antimicrobial exposure or inappropriate antithyroid treatment.
