  • <p>This article reports a case of acquired von Willebrand disease (AvWD) associated with IgA lambda multiple myeloma. No circulating inhibitor was detected. Multimeric analysis showed a decrease in all-sized von Willebrand factor (vWF) multimers with a type IA pattern. After DDAVP infusion, larger vWF multimers appeared transiently in plasma and were progressively cleared, suggesting increased removal from circulation. Indirect immunofluorescence using a monoclonal antibody demonstrated selective absorption of vWF into myelomatous plasma cells. The findings support a new pathophysiological mechanism of AvWD in multiple myeloma and potentially other lymphoproliferative disorders.</p>
