contents
This PETHEMA multicenter study analyzed 167 newly diagnosed PML/RARα-positive acute promyelocytic leukemia (APL) patients treated uniformly under the PETHEMA/LPA96 protocol to assess whether PML/RARα isoforms (bcr-1/L, bcr-2/V, bcr-3/S) are associated with baseline characteristics, response, and outcome. L-form accounted for 50%, V-form 5% and S-form 45%. V- and S-form cases showed a higher frequency of elevated WBC counts at diagnosis; S-form was significantly associated with microgranular M3v morphology and CD34 expression. Complete remission rates were similar across isoforms (overall 90%). Disease-free survival tended to be lower in V-form patients compared with L- and S-form groups, suggesting that V-form and S-form are linked to adverse baseline features, although confirmation in larger uniformly treated series is needed.
