contents <p>This study investigated serum iron levels in patients with oral submucous fibrosis (OSF) across different clinical stages and compared them with healthy individuals in Karachi, Pakistan. A total of 89 OSF patients and 89 matched healthy controls were evaluated. Serum iron was measured using a colorimetric method on a semi‑auto analyzer.</p> <p>The findings showed that OSF patients had significantly lower serum iron levels (111.66 ± 18.43 µg/dL) compared with healthy controls (122.38 ± 17.04 µg/dL). Serum iron levels progressively decreased with advancing functional stages of OSF, particularly in patients with severe mouth opening restriction and extensive fibrous bands. Although serum iron alone predicted OSF in only 8.5% of cases, predictability increased to 31.2% when combined with age, with age being the strongest individual predictor (28%).</p> <p>The study concludes that serum iron assessment should be incorporated into routine evaluation of OSF patients, especially those at high risk or showing declining iron levels. Early correction of iron deficiency—along with other micronutrients—may help slow disease progression and support overall management, potentially aiding in early detection of malignant transformation.</p>
